(1) The company has made a disclosure to the consumer in a separate document, that a report might be obtained for employment/tenant screening purposes
(2) Consumer has authorized in writing the procurement of report.
(3) Prior to taking any adverse action, based in whole or in part upon a report, I will provide to the consumer a copy of the consumer report and summary of the consumer's rights as prescribed by the Federal Trade Commission under 15 USC1681g
(4) Reports will not be used in violation of any applicable Federal or State law or regulation.
(5) According to the Fair Credit Reporting Act, reports will only be used for employment purposes.
